This Supplementary Form will be used by all of the Folio Trust Schools if a member of staff wishes to apply to another Folio Trust school using the 'children of staff' admissions band. Folio Staff are still to read the Admissions Criteria document to the School they are applying to and must ensure they are familiar with any terms or conditions of its use.
